I'm not a native speaker, but my take on your question is as follows:
If you want to literally say, "It is in your hands," then "Nasa kamay mo na ito."
However, if you are speaking idiomatically and meaning something like, "It's up to you," then "Bahala ka."
As always, I invite correction from those more fluent than I.

Ang kinabukasan nila ay nasa (mga) kamay mo. = Their future is in your hands.
Bahala ka sa gágawin mo. = What you'll do is up to you.
 
When you add "na" (already), it usually implies a handing over of something, literally or figuratively, to someone who'll be fully responsible for what to do with it.
 
Ang pera ay nasa (mga) kamay na ni John. = The money is already in the hands of John.
Nakausap ko na si Mary. Bahala ka na kung may gusto ka pang sabihin sa kaniya. = I already talked to Mary. It's already up to you if you'd still like to say something to her.
